Summary:
Wonderful activity. Very relaxing and the people running the show were very kind and attentive. Lunch is pretty decent and was in the middle of the sea on what seemed like a floating restaurant.
The only thing I would mention is (something previous reviews mentioned) they could use an upgrade of the condition of their mats, water beds, etc. Honestly, if they just improved the condition of the tools they used, they could raise the price quite a bit.

Summary:
The Dr. Fish Ocean Spa in Punta Cana, DR was DA BOMB!!! We started with the fish pedicure. Once we boarded the boat, we did a short yoga/pilates session followed by an ion spa detoxification on our feet. They gave us fresh watermelon juice, pineapple and nuts. The views were impeccable as we sailed into the ocean. We anchored briefly and were able to spend time in the water. Once we got out we had body scrubs and 20-30 min body massages. We sailed back to shore and had lunch. Must do!
